A visit built for endocrinologists
The visit is made of endocrinology sections: complaint, history, vital signs, endocrine status, diagnosis and plan. Every section also has a free-text field.
- History: diabetes type, year diagnosed, treatment, self-monitoring (meter or CGM), hypoglycaemia in the last 3 months, latest HbA1c; diabetes complications; thyroid conditions, surgery or radioiodine, latest TSH; menstrual and reproductive history
- Vital signs: blood pressure, heart rate, weight, height, waist circumference and capillary glucose; body mass index is calculated automatically
- Endocrine status: habitus and fat distribution; the thyroid (WHO goitre grade, nodules, consistency); thyroid signs including thyroid eye disease graded by EUGOGO; skin signs (acanthosis, lipohypertrophy); Cushingoid and other endocrine signs; a normal status fills in with one click
- Diabetic foot for each side: skin, deformity, 10 g monofilament, 128 Hz vibration, foot pulses, the IWGDF risk category and footwear
- Diagnosis with an ICD-10 code; a visit can't be completed without complaint and diagnosis. The plan records treatment targets (HbA1c, glucose, blood pressure, LDL, weight, TSH), the self-monitoring regimen, patient education (hypoglycaemia, sick-day rules, insulin technique, foot care) and complication screening that is due
FINDRISC and measurements — over time
The FINDRISC scale scores type 2 diabetes risk during the visit; the result is saved to the patient's chart and plotted over time. Blood pressure, heart rate and body mass index measured at visits are plotted on the chart too. Doctors keep their own templates and copy a section from the previous visit in one click; visit summaries and referral letters come out as PDFs.
A questionnaire before the visit
If your clinic switches it on, the patient is invited 72 hours ahead to fill in an endocrinology questionnaire in the portal — about 5 minutes: complaint and symptoms, diabetes (type, treatment, home glucose checks, hypoglycaemia, latest HbA1c), the thyroid, past illnesses, family history, periods and pregnancy for women, medicines and allergies. If the patient reports signs of ketoacidosis or a neck swelling that makes breathing hard, the portal tells them straight away to call 112; an inflamed foot wound, to call the clinic that day. Nobody reads the questionnaire before the visit — the doctor sees the answers during the visit and moves them into complaint and history in one click; what the doctor wrote is never overwritten.
Between visits: glucose, blood-pressure and weight diaries
When the visit is completed, the doctor shares a plan with the patient: the follow-up date, tasks and warning signs. There are ready-made lists for low and high blood sugar in diabetes, the diabetic foot and the thyroid, so the patient gets in writing when to call 112 and when to contact the clinic. At the doctor's request the patient keeps a blood-glucose diary (linked to meals), a blood-pressure or a weight diary in the portal; on a very low or high glucose reading the portal tells them on the spot what to do. The patient decides whether to share a diary with your clinic; shared readings appear in the "Home data" tab. Nobody watches them in real time — the doctor reviews them at the visit.
For the owner: cash desk, insurance, follow-ups
Diabetes and thyroid disease need follow-up for years. Unbooked follow-ups land on a recall list, and a freed slot is offered to the waitlist. Allergies and drug interactions are checked automatically when a prescription is written — including what other clinics prescribed — and the doses a patient logs from medication reminders show on the chart for medicines your clinic prescribed. Cash-desk shifts, discount limits, voluntary insurance registries and doctor commissions live in the same system.
Frequently asked questions
Which endocrinology scales and gradings are included?
FINDRISC (type 2 diabetes risk) is scored during the visit and plotted over time. In the endocrine status the doctor picks the WHO goitre grade, the EUGOGO severity of thyroid eye disease and the IWGDF risk category for the diabetic foot.
Will the doctor see the glucose a patient measures at home?
Yes, if the patient shares their glucose diary with your clinic. The doctor asks for the diary in the care plan; the patient decides whether to share it. Readings appear in the patient's chart. It is not an emergency channel — nobody watches them in real time.
Does it connect to glucose meters, CGM sensors or a lab?
No, Anamnez does not connect to devices. The patient enters glucose readings in the diary themselves. Patients can send you results from other labs (HbA1c, TSH and more) through "My files" in the portal (PDF, PNG, JPEG).
